Welcome To Oyster Point Family Practice

Welcome Letter - If a new patient appointment is scheduled far enough in advance, a welcome letter is sent to the patient.  Along with the letter, is a packet of forms that need to be filled out prior to the first visit.

 

New Patient Packet - This packet includes the demographic form, a prior medical history form and a HIPAA form.  As an explanation of the form, the "guarantor" is the person responsible for the bill after all insurance claims have been paid or denied.

 

To facilitate a timely and beneficial appointment, it is requested that a new patient completes these forms prior to his arrival at Oyster Point Family Practice.

 

Please arrive 20 minutes early for your first visit so that you may be registered as a new patient and will not cause the physician to run late. 

 

Please bring your pictured ID, insurance card(s) and any medications and supplements, in their bottles, that you are currently taking. 

 

If your insurance has a copay, you will be expected to pay it at the time of your visit, unless other payment arrangements have been made with our business office.  For your convenience, we accept VISA, MasterCard, Discover and American Express, as well as checks and cash.

 

Canceled or Missed New Patient Appointments - If you are unable to keep your appointment, please cancel no later than 24 hours in advance.  You may call our office during normal business hours at (757) 873-2000 or after hours you may leave a message at (757) 223-4091.   Please remember if you do not cancel or show up for your new patient appointment, we will not be able to reschedule you.
