How Do I Get an Insurance Referral?

Our Doctor, Nurse Practitioners, and Physician Assistant often make the decision to refer a patient to a specialist for more specific treatment of a condition or disease.  Some insurance companies require the patient to obtain a referral from their primary care physician prior to their initial visit to a specialist.  If your insurance requires a referral, our office will process this referral for you and fax a copy to the specialist you will be seeing.  We will also fax any pertinent office notes, labs, tests, procedure notes and demographic information to the specialist's office prior to your first visit.

Occasionally, continued treatment by a specialist may be necessary and a referral needs to be renewed.  If you need an insurance referral to be renewed, it is your responsibility to contact our office in a timely manner.  We request that you allow two weeks for your referral to be renewed.  When requesting a referral, please provide the first and last name of the specialist you will be seeing, the specialty, the phone and fax numbers to the specialist's office and the date of your appointment along with a number where you may be reached.  If our referral department has any questions regarding your request, they will contact you at the number you provide.

We will fax the referral to the specialist's office, but will not provide notes or test results, unless specifically asked to do so.
