Patient Portal  Information

Oyster Point Family Practice now has a Patient Portal through Follow My Health.  This portal allows our patients to have 24/7/365 access to their medical records here, at Oyster Point Family Practice.  It also has many other features and benefits available to our patients, some of which are listed below. 


  • Send secure messages to Provider at any time - day or night
  • Request medication refills without making a phone call
  • Receive appointment reminders via email or text messaging
  • View/Print Lab or Test results
  • Setup healthcare reminders for medication refills or scheduling of necessary tests or appointments
  • Utilize your "Health Journal" for notes, reminders, keep a pain journal, etc...
  • Install one or more apps in your "Apps Center"
  • Use the "My Charts" App to graph any data from your chart - Ex: glucose monitoring, A1C levels, weight management, etc...
  • Pay your bill online
  • Access a multitude of diagnosis-specific patient education material
  • Plus much more...

The portal is a secure site and you must be sent an invitation to register for the portal access.  After your invitation is received, you must create a unique username and password in order to have access to your medical records.  The portal will allow access to ONLY one person's records with each account, so families with multiple patients will have multiple accounts, one for each patient in the home - with every patient having access to his or her record ONLY.  In the case of minors, there are different rules.  Please keep in mind that a minor begins at age 14 for medical privacy in the Commonwealth of Virginia.
