If you are already a patient of Oyster Point Family Practice and you have been seen within the last year, and you would like to sign up for the OPFP Follow My Health Patient Portal, please complete the form below and click Send.  (Please include the email address that you want the invitation sent to on the form, does not necessarily have to be the one that you will use for the portal.)
